Title: Lead Family Case Manager

Location: Orlando, FL
Required Experience: Five years of related child welfare experience (beyond probationary period)

Degree Type/Area: Bachelor's degree in social work or related degree required. Master's degree in social work or related degree preferred. Immediate enrollment in master's program if selected.
Certification: Child Welfare professional required.
Description:
Lead Family Case Managers provide an array of services to children and families that represent best practices. This position focuses on preventing inappropriate out-of-home placement, strengthening and improving family functioning, and ensuring child safety. Lead Family Case Managers act as team leader in Family Case Manager Supervisor's absence and reports to Family Case Manager Supervisor. Lead Case Managers will have capped case load, which is determined at a service center level by the Director of Operations.
Specific Responsibilities:
- Provides in-home services to families and children based on identified needs:
- Assesses child and family safety and risk.
- Develops individualized case plans.
- Oversees case responsibilities.
- Assists families with identifying and removing barriers that prevent children from remaining or being returned home.
- Conducts home visits based on individual family need.
- Maintains accurate home-visit notes.
- Refers families to community resources and provides linkages with necessary treatment providers.
- Completes required written documents in a timely manner.
- Works with families to ensure permanency for clients.
- Completes various outcome reports as required.
- Attends court hearings, case reviews, staffings and meetings.
- Ensures that required documents are in case files.
- Ensures unusual incidents reports are completed accurately and promptly.
Supervisory Duties:
- Attends court in Supervisor's absence.
- Attends supervisory trainings.
- Demonstrates a proficiency in HSN, this includes an ability to run HSN reports and correct HSN errors.
- Reviews and provides feedback to Family Case Managers on Judicial Reviews, Initial Family Assessments and Case Plans prior to submission to Supervisor.
