Title: Placement Counselor
Post Date: June 3, 2008
Location: South Miami, Fla.
Required Experience: Four years related experience

Certification: PDC certified
Description:
Must be able to communicate effectively with all levels of management. Four years experience in child welfare system and or case management required. Must be proficient in software skills and demonstrate a strong ability to solve problems effectively. Bilingual preferred.
Specific Responsibilities:
- Completes monthly foster care home-visits.
- Completes and submits placement referrals to identify appropriate placement for children in foster care.
- Reviews psychological evaluations, psychiatric evaluations, psycho-educational evaluations and other assessments to ensure recommendations are followed through by the clinical and case management team and to assist in determining the most appropriate placement that will meet the client's needs.
- Coordinates and reviews referrals with the multidisciplinary/foster care team to identify and match prospective foster families with clients in need of placement.
- Gathers, completes and submits all documentation needed for specialized and therapeutic placements through such programs/agencies as RGC, UMT and Developmental Services.
- Attends reviews and staffing meetings regarding progress and follow-up for children in specialized placements.
- Meets with outside agencies and/or attends intake interviews to discuss possible placements within those agencies.
- Maintains a good working rapport and communication with all outside provider agencies.
- Assists in the coordination and completion of referrals for Therapy and Behavioral Services to preserve and maintain current placements.
- Calls prospective foster families to discuss clients' potential foster placements.
- Maintains contact with foster parent after a placement has been made to ensure a smooth transition for both the client and the foster family.
- Identifies emergency placements with the multidisciplinary/foster care team when a placement is disrupted.
- Maintains on-call availability 24 hours/7 days a week.
